Use:

Same content as print referral cards. Content in Word template can be copied and pasted into body of email and should retain its formatting. Or, if formatting doesn’t work well, the plain text version can be used to cut and paste plain text into the body of an email. The .eml file can be opened with Outlook or other email platforms.

Can be used to refer during or after telehealth appointments by referring clinicians, their staff, or COZI team partners.

Can be used if no referral card was given or it was lost.
